Patient 2. MRI brain scans revealed diffuse bilateral pachymeningeal thickening that was T1-weighted isointense (a, axial, grey arrowhead) and demonstrated intense contrast-enhancement (b, white arrowhead). Cervical spinal imaging also revealed an intradural-extramedullary lesion at the C4/5 level posterior thecal sac that was also T1-weight isointense (c, sagittal, white arrow) and exhibited homogeneous contrast enhancement with a dural-tail sign (d). The spinal lesion caused significant cord compression (T2W, sagittal). After 2 months of prednisolone and azathioprine, a repeat MRI scan showed no recurrent lesion (f, contrast-enhanced T1W)
